Job Summary
Provide direct personal care to terminally ill patients, including assistance with hygiene, grooming, toileting, safe mobility, meal preparation, and light housekeeping. Observe and report changes in patient conditions, offer emotional support to patients and families, and maintain accurate visit documentation. Practice infection control and follow safety protocols while attending team meetings and Quality Improvement activities. Requires an active CNA license, CPR certification, driver's license, and six months of direct patient care experience.
Required Qualifications
- Active CNA license in the state of employment
- Current CPR certification
- Driver's license and reliable vehicle with insurance
- Six (6) months of direct patient care experience
- Comfortable working in home settings, hospice facilities, or long-term care environments
- Demonstrated compassion, patience, and ability to handle emotional situations
- Willingness to work independently and adapt to changing patient needs
- This is a safety-sensitive position
- Employee must meet minimum requirements to be eligible for benefits
- Employee must meet state specific requirements

Desired Qualifications
- Experience in home health, hospice, palliative care, or oncology
- Prior roles in hospital nursing, ICU, geriatrics, med-surg, ER, telemetry, or nursing homes
- Exposure to end-of-life care, wound care, or admissions support
