Job Summary
Provide outpatient primary care services to patients across Pittsburgh, Pennsylvania, including Eastern Neighborhoods, Westmoreland County, Downtown, and Squirrel Hill. Manage clinical responsibilities within a value-based care model emphasizing quality outcomes, preventative care, and long-term wellness. Coordinate with extended care teams comprising mid-level support, behavioral health consultants, social workers, registered dietitians, and certified diabetes educators. Maintain board eligibility or certification, hold a Pennsylvania license, and ensure DEA and CPR certification prior to employment.
