Job Summary
Provide outpatient primary care services in Pittsburgh, Pennsylvania, serving diverse neighborhoods including Eastern Neighborhoods, Westmoreland County, Downtown, and Squirrel Hill. Manage patient wellness within a value-based care model that emphasizes preventative care and quality outcomes. Coordinate with extended care teams comprising mid-level support, behavioral health consultants, social workers, and registered dietitians. Deliver correctional medicine services at the Allegheny County Jail. Maintain board eligibility or certification and hold a Pennsylvania license, DEA, and CPR certification. Required Qualifications
