Job Summary
Conduct outpatient primary care for patients across Pittsburgh, Pennsylvania, including Eastern Neighborhoods, Westmoreland County, Downtown, and specific facilities like the Allegheny County Jail. Deliver value-based care focused on quality outcomes, preventative services, and long-term wellness while utilizing extended care team support from mid-level providers, behavioral health consultants, social workers, dietitians, and certified diabetes educators. Maintain board eligibility or certification, hold a Pennsylvania license, and ensure DEA and CPR certification prior to employment.
