Job Summary
Aging Care Manager — Part-time, PRN opportunity in Minneapolis, MN for a gerontology professional to assess, plan, coordinate, and monitor care for aging clients. Responsibilities include client assessment, care planning, coordinating services (home health, transportation, meal services), advocating for client needs, connecting clients with community resources, educating families, and documenting interactions. Requires Master’s degree in a related field, active professional licensure, and 5+ years of direct care or care-management experience. The role involves in-person, remote, and home visits, with a flexible schedule and case-based assignments. Salary varies by degree and experience ($50-$70k for Bachelor's level; $70-$135k for Master’s; $130-$165k for PhD/Doctorate), final rate determined during interview; part-time, hours not guaranteed. Required Qualifications
- Master’s degree in Social Work, Nursing, Gerontology, Occupational Therapy or related field
- Professional Licensure in good standing (RN, NP, LMSW or other nursing/social worker license)
- Minimum of 5 years of direct care experience in care management, home care, acute care, and/or a rehabilitation setting
- Strong knowledge of aging issues, healthcare systems, and community resources
- Experience working closely with families & professional providers
- Ability to work independently and as part of a multidisciplinary team
- Excellent written and oral communication, interpersonal, and organizational skills
- Comfort and quick learning curve with technology on day-to-day tasks (i.e. Microsoft Office, Salesforce)
- Valid driver's license and reliable transportation
