Job Summary
Conduct habilitation, companion, and transportation duties for individuals with intellectual and behavioral disabilities in their homes or community settings. Provide a safe, clean, and therapeutic environment while encouraging daily living skills development per individualized service plans. Organize and participate in leisure and recreation activities, assisting with physical needs such as wheelchair transfers or incontinence care. Maintain ongoing communication regarding program planning and implement delegated tasks by Family Support Coordinators. Drive extended periods to community activities using a personal vehicle where required, ensuring all transport and EVV access needs are met.
Required Qualifications
- 18 years or older
- Valid Vehicle Insurance and Registration (100/300 Liability is required) If being reimbursed for transporting Individuals
- Computer knowledge; Basic computer skills
- Physical ability to perform all duties associated with this position
- Properly preform lifting and carrying techniques. (Lift 25lbs)
- Valid U.S. Driver's License/ID
- Ability to function in a team environment
- Ability to function independently
- Ability to be organized
- Ability to follow directions
- Ability to follow through with assigned tasks in a timely fashion
- Ability to work well with individuals
- Physical ability to assist any physical needs in the community (i.e. assist w/ wheelchair, assist w/ walkers, assist with incontinence accidents etc.)
- Ability to drive for extended periods of time
- Ability to be in the community for extended periods of time
- Ability to independently research community activities appropriate for the individuals they are working with
- Have a device to access and use EVV
