Job Summary
Conduct comprehensive psychiatric assessments, treatment planning, and crisis support for clients in home, community, and residential settings. Provide intake, triage, and case management to reduce illness impact and enhance independence for older adults. Deliver discipline-specific advice to the APATT team while reviewing risk factors across the continuum of care. Requires postgraduate qualifications, AHPRA registration, and five years of clinical experience in mental health and aged care. Full-time permanent role at St Vincent's Health Australia Melbourne with salary between $121,274 and $130,603 per annum.
Required Qualifications
- Relevant postgraduate qualifications
- Current registration with the Australian Health Practitioner Regulation Agency (AHPRA)
- 5 years post qualification clinical experience within Mental Health
- care of older adults with clinical services experience including case management and care coordination
- Relevant knowledge of the Victorian Mental Health and Wellbeing Act 2022 and other relevant legislation
- A valid Nationally Coordinated Criminal History Check, or willing to obtain
- You may therefore be required to undergo mandatory immunisations/vaccinations (including flu vaccinations)
