Aged Care Comprehensive Assessor

Job Summary
Conduct thorough Aged Care Assessments to identify the needs of older Australians, and facilitate referrals to essential services to support independent living. Promote retention and maximization of function and capability in daily living, engaging families and carers as key stakeholders. Ensure assessments align with Department of Health contractual requirements. Travel daily within the service area to meet business and client needs. Join a multidisciplinary team with extensive training, flexible working arrangements, and opportunities for career development. This role supports a diverse range of health disciplines and requires proficiency with Word, Excel, Outlook, and client management systems, plus a valid Australian driver’s licence and an insured vehicle.
Required Qualifications
- At least 1 year working with older Australians, including client assessments and/or service delivery
- Tertiary education in a health-related discipline together with professional registration with relevant association (e.g. AHPRA)
- Proficiency in Word, Excel, Outlook, and client management systems
- Current Australian driver’s licence, comprehensively insured vehicle, and the ability to travel within your service area
