Aged Care Clinical Assessor

Conduct thorough Aged Care Assessments to identify the needs of older adults and facilitate referrals to essential services supporting independent living. Actively promote the retention and maximization of function and capability in daily living while working closely with families and carers as key stakeholders. Ensure all assessments are conducted in line with Department of Health contractual requirements. Daily travel is essential across the Far North Queensland service area to deliver support where needed most. This role requires tertiary education in a health-related discipline, current unrestricted General Registration with AHPRA, and at least one year of aged care experience including client assessments.
