Job Summary

Conduct direct in-person patient care during 2pm-12am Monday-Thursday after-hours shifts, including assessments, admissions, routine visits, death visits, and live discharges. Perform initial and ongoing assessments of physical, emotional, psychosocial, and spiritual needs while developing and revising Hospice Plans of Care in collaboration with physicians. Support the Interdisciplinary Group through chart reviews, plan updates, and interdisciplinary communication, ensuring seamless coverage and continuity for patients and families. Maintain accurate clinical documentation, assist with scheduling modifications, and lead field staff through supervisory visits and trainings.

Required Qualifications

  • Current RN license in the state of employment
  • Graduate of an accredited nursing program
  • Minimum one year of RN experience in acute care, oncology, long-term care, hospice, or home health
  • Must meet any state-specific requirements
  • Active RN license
  • CPR certification (in-person)
  • Valid driver's license
  • Reliable transportation
  • Current auto insurance
