Job Summary
Answer incoming phone calls after hours and on weekends to triage patient needs, escalating to clinical staff as required. Coordinate care with physicians and the Interdisciplinary Group, ordering medications and durable medical equipment while performing in-person nursing visits throughout San Diego County when visit needs arise. Clearly document all interventions and visit notes in patient records. Requires an active California LVN license, reliable transportation for county-wide travel, and strong clinical judgment in a remote, after-hours setting. Required Qualifications
- Active and unencumbered LVN license in the State of California
- Reliable transportation
- Ability to travel throughout San Diego County
- Strong triage, assessment, and clinical judgment skills
- Excellent verbal and written communication skills
- Ability to document clearly under time pressure
- Comfort working independently in a remote, after-hours, on-call capacity
Desired Qualifications
- Prior hospice, home health, or triage experience
