Aftercare Worker

Job Summary
Work with families who have achieved safe case closure after receiving formal in-home child welfare services. Responsibilities include utilizing the Strengthening Families framework, maintaining confidentiality, ensuring compliance with HIPAA, and completing documentation in accordance with guidelines. Preferred candidates will have knowledge of available SWAN services, effective communication skills, and the ability to foster relationships with clients and co-workers. Candidates should be prepared to transport clients and work a flexible schedule to meet the needs of families.
Required Qualifications
- Bachelor’s degree in Social Work or related field including sociology, psychology, counseling, criminal justice, or education
- Two years experience in Child Welfare or a related field
- Valid, timely Child Abuse, Criminal History, and FBI clearances that meet all regulatory requirements
- Certification as a Direct Service Worker according to the requirements set out by the Commonwealth and the Office of Children Youth and Families in regulation and bulletin
- Good oral and written communications skills in English
Additional Requirements
- Must possess a valid PA. driver’s license and a good driving record
- Must have own reliable vehicle with current registration and insurance
