Job Summary
Oversee discharge and continuing care planning for patients transitioning to Seafield sober homes, ensuring smooth transitions to the next level of care. Conduct discharge planning and document in EMR while coordinating transportation, referrals, and outpatient appointments. Liaise with case managers and next care facilities to manage admissions, discharges, and house census. Screen calls for housing criteria and maintain weekly reports. Perform office duties and complete monthly housing reports as assigned. This role requires proficiency in EMR and Microsoft Outlook, with a focus on affirming care for LGBTQ+ clients.
Required Qualifications
- Minimum one year of discharge planning experience
Desired Qualifications
- Bachelor's Degree preferred
- CASAC and supervisory experience preferred
