Job Summary
Provide information, referrals, and follow-up as specified in treatment plans for individuals served. Complete transition plans and required documentation while maintaining knowledge of levels of care criteria and available resources. Collaborate with treatment teams, families, internal leadership, and external agencies to ensure successful referrals to the next level of care. Gather information for EIM/ESM enrollments and facilitate diverse treatment groups as needed. Transport individuals served and recognize specific concerns requiring supervisor guidance or emergency contact.
Required Qualifications
- High School Diploma or GED
- Minimum of 1 year of relevant experience in behavioral health or community support services
- Valid driver's license
