Aftercare Coordinator

Coordinate each patient's discharge plan and next level of care by collaborating with clinical staff, the Outbound Team, and external agencies like the VA and probation officers. Facilitate admissions-related needs by obtaining consents, identification, and financial documentation while referring administratively discharged patients to alternative providers when necessary. Maintain accurate documentation of employment, legal, and medical interactions in Kipu and share key updates via Teams and Salesforce. Engage families throughout the discharge process, provide timely updates, and support patient retention initiatives including AMA blocking efforts. Participate in weekly case consultations to ensure seamless transitions and address all post-treatment needs with care and accuracy.
