Job Summary
Aftercare Case Management Specialist responsible for coordinating services for children and families after reintegration, using advanced knowledge in child welfare to develop individualized service recommendations, facilitate planning with families and case teams, prepare reports for court-involved cases, and maintain on-call accessibility to stakeholders. Role supports KS Foster Care with collaboration across team members, ensuring safe home transitions and ongoing support, backed by trauma-informed, evidence-based practices and adherence to DCF policies. Requires experience with children and families, relevant degrees (BSW/MSW), and state licensure where applicable.
Required Qualifications
- Bachelor’s degree in social work or other human services related field
- Master's degree in social work or other human services related field and licensed by the Behavioral Sciences Regulatory Board to practice in Kansas are preferred
- At least 21 years of age and pass background check, physical, and drug screening
- A valid driver’s license in the state you reside in, proof of current vehicle insurance, and reliable transportation
