Job Summary
Conduct nursing assessments, make visits, and coordinate patient/caregiver care while following established plans of care. Perform emergency triage by telephone and home visits, providing interventions consistent with the plan and identifying new needs. Develop, implement, and evaluate plans of care that incorporate therapeutic, preventive, and palliative actions, including patient education. Maintain a liaison with healthcare providers, communicate with the interdisciplinary team, and refer psychosocial needs to the Medical Social Worker. Document clinical records, medications, and mileage accurately while adhering to infection control and universal precautions. Maintain current licensure, CPR certification, and CEUs, and stay abreast of current trends in the field.
Required Qualifications
- A Diploma or Associates Degree in nursing
- Bachelor's Degree
- A current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN)
- Current CPR
- Minimum 2 years related experience
- Experience in direct care of patients with life limiting illnesses
- Prior experience in working in an environment which focuses on the pediatric population
- BCLS (CPR) training for both adult and child/infant
- Computer proficiency
