Job Summary
Provide continuing care to patients and families as requested while assessing, planning, implementing, and evaluating nursing services consistent with the plan of care. Communicate with attending physicians and community resources to report changes and coordinate optimal care, and maintain regular communication with Team Leaders regarding patient status or unusual issues. Ensure all activities comply with agency policies, procedures, and applicable federal and state regulations. Work every Friday through Monday from 3:30pm to 12:00am in Marin and Sonoma Counties.
Required Qualifications
- One year experience as a professional nurse in the past three years
- Bachelor's degree in Nursing from an accredited School of Nursing
- Current license as a registered professional nurse in the State of California
- Must be available for Friday, Saturday, Sunday & Monday shifts
- Must be available for 3:30pm - 12:00am shifts
- Must be able to work in Marin County & Sonoma County
Desired Qualifications
- Experience in acute care/ home health and/or hospice setting
- Public Health Nursing certification
- HPNA certification
