Job Summary
After Hours LPN in Hospice providing in-home nursing care to patients in the Champaign area. Responsible for delivering professional nursing care within the designated scope of practice, assessing patient status, initiating and revising plans of care with the IDG/case-management team, administering prescribed medications, documenting patient information accurately, communicating with physicians and community health personnel, coordinating equipment and services, and pursuing ongoing professional development. Must travel locally to patient homes, work evening shifts (8 PM–8 AM) with alternating 4-day/3-day weeks and weekend rotation, and uphold high-quality patient care with a focus on comfort and family support.
Required Qualifications
- Graduate of an accredited school of Nursing
- LPN licensure in the state of practice
- 1+ year Hospice, home health or related experience preferred
