After Hours LPN

Provide direct patient care as an LPN on the After Hours hospice team, under RN supervision, following individualized care plans and ensuring comfort and support. Document care in the EMR at bedside, educate patients and families about diagnoses and self-care, manage equipment and supplies, monitor and adjust pain and symptoms in collaboration with the RN/provider, and participate in interdisciplinary team meetings and quality initiatives. Travel throughout the full service area for in-person patient care and adherence to policies and protocols. Strong communication, organization, critical thinking, and reliable transportation are required; license in Virginia and CPR certification are mandatory.
