Job Summary
Provide assessments, mental health evaluations, family-individual-group therapy, plan development, case consultation, crisis intervention, discharge placement, and assist clients in meeting basic needs and symptom management. Include pre-placement visits, transportation coordination, and participation in on-call shifts; conduct crisis-focused assessments and stabilization, and determine appropriate levels of care in collaboration with emergency services. Maintain confidential client information, ensure accurate documentation and billing, and engage in field-based services as required by program and client needs. Proficiency with Microsoft Office and EHR systems; bilingual capabilities preferred. Requires Master’s degree and CA licensure (MFT/LCSW/LPCC); valid CA driver’s license; ability to coordinate care across health, educational, social, vocational, and housing services. Shift coverage includes after-hours calls and outreach; participate in program meetings and adhere to equal opportunity employment policies.
Required Qualifications
