After-Hours Admissions Nurse

Make in-person nursing visits to patients as assigned, providing continuing care in compliance with agency policies and applicable regulations. Use the nursing process to assess, plan, intervene, and evaluate based on patient and family needs. Provide effective management of symptoms with prompt escalation to providers as indicated. Coordinate patient and family services and prioritize needs with the transdisciplinary team or outside providers. Cover after-hours staffing needs and process admissions for eligible hospice patients in coordination with the Team Manager. Schedule runs Monday through Friday from 4:00pm to 12:30am.
