Job Summary
Support individuals with intellectual and developmental disabilities in a home setting by preparing nutritious meals, managing daily self-care, and ensuring a safe living environment. Coordinate medical, therapy, and recreational activities while attending treatment team meetings to address personal goals outlined in the Person Centered Plan. Provide transportation, facilitate communication regarding service progress, and honor the individual's cultural preferences and choices. Maintain the home in good condition and assist with rest, leisure, and education activities tailored to the person's abilities.
Required Qualifications
- HS Diploma/GED
- 18 years old or older
- Documentation of home ownership: current mortgage statement or renter's lease
- Proof of homeowner's renter insurance or personal property insurance
- Valid Driver's License
- Ability to pass a criminal background check, Healthcare Registry and Urine Drug Screen
- Work as an Independent Contractor
