Job Summary
Conduct 24/7 care for individuals served in a family-based setting, blending into the host household while participating in team meetings and carrying out individual goals. Submit weekly data reports to the Quality Program and maintain a vehicle with active full insurance for transportation, ensuring wheelchair accessibility when required. Complete RELIAS training, CPR/First Aid, Medication Administration, and Behavior Management programs on time, holding clear driving and criminal records. Work as a back-up or PRN provider under an agreement with Skill Creations Inc., receiving a daily rate plus a monthly Room and Board supplement.
Required Qualifications
- Must live in a dwelling that passes Organization, State and LME-MCO regulations
- A Vehicle for transportation of the individual with active full insurance (collision and liability)
- Clear Driving and Criminal Records
- Successful completion (on time) of RELIAS training (provided by Skill Creations Inc.)
- Successful completion (on time) of CPR/ First Aid, Medication Administration and Behavior Management Programs
