Job Summary
Advanced Practitioner provides primary medical care with a strong focus on preventive care and integrated wellness in collaboration with the mental health team. Responsibilities include outpatient primary care evaluations, diagnosis and treatment of acute/chronic diseases, documentation and case management across outpatient, telehealth, and mobile clinic sites throughout Western North Carolina; develop and oversee patient care plans; participate in monthly quality improvement meetings; supervise volunteers/university students as needed; collaborate with the clinical team to provide integrated healthcare to uninsured/underserved populations; work under Medical Director supervision and support outreach medical services.
Required Qualifications
- Licensed/certified Nurse Practitioner or Physician Assistant in the state of North Carolina
- Advanced English and Spanish fluency
- Willing to participate in outreach events in surrounding counties, as needed
