Job Summary
Deliver senior primary care to Medicare Advantage, Medicare, and Medicaid populations while managing chronic disease and preventative care within a value-based model. Maintain active Texas APRN licensure and DEA eligibility, utilizing EMR systems to document visits and build patient relationships. Work Monday through Friday with a guaranteed work/life balance, no on-call rotation, and supplemental bonus opportunities. Required Qualifications
- At least 2-3 years' experience in the primary care/value-based care setting, working with Medicare Advantage, Medicare, and Medicaid patient populations
- Active and unrestrictive Texas APRN license
- DEA license or eligible to obtain it within 2 months of date of hire
- Strong understanding of chronic disease management, preventative care, and quality metrics in a value-based care model
- Excellent documentation and patient relationship-building skills
- Proficient with EMR systems
