Job Summary
Deliver primary care and other services to seniors and families in comfortable, convenient medical centers. Manage chronic disease, preventative care, and quality metrics within a value-based care model while building strong patient relationships. Maintain excellent documentation proficiency using EMR systems, with eCW proficiency preferred. This role requires at least two to three years of experience in primary care or value-based settings with Medicare Advantage, Medicare, and Medicaid populations. Candidates must hold an active, unrestrictive Texas APRN license and DEA license or eligibility to obtain it within two months. The position offers Monday through Friday hours with a great work/life balance, no on-call schedule, and bonus opportunities.
Required Qualifications
- At least 2-3 years' experience in the primary care/value-based care setting, working with Medicare Advantage, Medicare, and Medicaid patient populations
- Active and unrestrictive Texas APRN license
- DEA license or eligible to obtain it within 2 months of date of hire
- Strong understanding of chronic disease management, preventative care, and quality metrics in a value-based care model
- Excellent documentation and patient relationship-building skills
Desired Qualifications
- Proficient with EMR systems (eCW a plus!)
