Job Summary
Manage primary care services for Medicare Advantage, Medicare, and Medicaid populations with 2-3 years of experience in value-based care settings. Deliver personalized, preventative care while maintaining strong documentation and patient relationship-building skills within EMR systems. Lead in geriatrics or internal medicine with full autonomy to influence staffing decisions and make a major impact on patient quality of work. Enjoy a standard day shift from 8am to 5pm Monday through Friday with no scheduled on-call, supported by adequate staffing levels and a strong leadership team.
Required Qualifications
- Have at least 2-3 years' experience in the primary care/value-based care setting, working with Medicare Advantage, Medicare, and Medicaid patient populations
- Must have an active and unrestrictive Florida APRN license
- Must have DEA license or eligible to obtain it within 2 months of date of hire
- Strong understanding of chronic disease management, preventative care, and quality metrics in a value-based care model
- Excellent documentation and patient relationship-building skills
- Proficient with EMR systems
- No weekends
- In-person
- Private practice
