Job Summary
Provide primary care services as an Advanced Practice Registered Nurse/Nurse Practitioner in assisted living facilities across the Albany region. Conduct comprehensive health assessments, examinations, and medication reviews; develop and implement individualized treatment plans; manage chronic diseases and acute conditions; coordinate care with facility staff, specialists, and family members; provide preventive care and health education; maintain accurate clinical documentation; ensure compliance with Medicare/Medicaid and regulatory guidelines. Travel between Hoosick Falls, Cambridge, and Troy, NY with options for part-time (2–3 full days/week) or full-time (4–5 full days/week) schedules. Monday–Friday, no nights or weekends. Required Qualifications
- Master’s Degree in Nursing from an Accredited NP Program
- Active New York State Nurse Practitioner License
- Active DEA Registration
- Credentialed with Medicare and Medicaid (Required)
- Ability to Practice Independently (Minimum 3,600 Clinical Practice Hours in Accordance with NYS Requirements)
- Primary Care Experience Required
- Strong Clinical and Communication Skills
