Job Summary
Provide in-home and telehealth advanced practice care to patients with complex medical needs. Deliver comprehensive health assessments, manage urgent and chronic conditions, and collaborate with a multidisciplinary team to ensure integrated, high-quality care. Perform medication management, address social determinants of health, participate in team huddles and clinical partnership meetings, and ensure accurate documentation and coding. Participate in after-hours on-call and clinical cross-coverage as needed. Requires Florida-licensed APNs or PAs with DEA and state CS registration; geriatrics/palliative training preferred; travel to patient locations and telehealth delivery are integral to the role. Required Qualifications
- Licensed Nurse Practitioner (ANCC or AANP certification) or Board-Certified Physician Assistant with active licensure in Florida
- Experience providing care to aging adults or individuals with complex medical needs
- Proven experience delivering in-home and virtual care via telehealth
- Active DEA licensure and state-specific controlled substance registration, as applicable
- Training or certification in geriatrics and/or palliative care is highly preferred
- Previous experience in managed care or value-based healthcare delivery is a plus
- A passion for working with underserved populations and addressing healthcare inequities
- Average to advanced computer and software skills
- Ability and means to travel to locations as needed; proof of liability and property damage insurance on vehicle used is required
- Driver requirements:Licensed for a minimum of 5 years; no major violations
Desired Qualifications
