Job Summary
Provide high quality primary and palliative care to homebound patients in the Tulsa region. Conduct physical examinations, obtain and update medical histories, and develop care plans including impressions, diagnoses, and treatments. Prescribe medications and necessary interventions, order diagnostic testing, and coordinate with office staff and clinical operations to support a patient-centered panel. Foster trust with patients, families, and caregivers during visits and manage care across remote monitoring and chronic care programs. Must be Board Certified NP or PA in Oklahoma with 1-2 years of primary care experience in geriatrics; hospice experience preferred. Strong clinical skills, ability to work independently, and excellent communication are required. Location: Tulsa, OK; Full-time position with competitive compensation and benefits.
Required Qualifications
- Board Certified NP or PA with valid Oklahoma license/certification
- 1-2 years of primary care experience with geriatric patients
- Ability to work independently and problem-solve
- Strong clinical skills and patient-centered care
- Positive demeanor and effective communication with patients, families, and staff
- Experience in home-based primary/palliative care or hospice preferred
- Capacity to assess, diagnose, and develop treatment plans; order diagnostic tests; and prescribe medications/interventions
