Advanced Practice Provider (Nurse Practitioner or Physician Assistant)

Job Summary

Conduct pre-employment physicals, annual wellness assessments, and acute illness evaluations while managing chronic conditions using evidence-based guidelines. Develop individualized treatment plans, order and interpret diagnostic studies, and counsel patients on lifestyle modifications and disease prevention. Coordinate referrals, maintain accurate electronic medical records, and support occupational health services including respirator fit testing and hearing evaluations. Collaborate with medical leadership and interdisciplinary teams to promote wellness initiatives and achieve better health outcomes. Requires an active NP or PA license, one year of relevant experience, and comfort with pediatric patients in a fast-paced outpatient environment.

Required Qualifications

  • Board Certified Nurse Practitioner (NP) or Certified Physician Assistant (PA)
  • Active and unrestricted state license
  • Minimum of one year of experience in one or more of the following settings: Family Medicine, Internal Medicine, Urgent Care
  • Current BLS certification
  • Strong written and verbal communication skills
  • Ability to work efficiently in a fast-paced outpatient environment
  • Commitment to delivering patient-centered care
  • Comfortable caring for pediatric patients in a clinical setting beyond routine wellness visits, milestone assessments, and vaccine management

Desired Qualifications

  • DEA license
  • Experience in occupational health, employee health, or wellness-focused clinical settings
  • Experience working across multiple patient care environments
  • Experience collaborating with multidisciplinary teams and organizational stakeholders
  • Local ties to the Cumming, GA area or surrounding communities
  • Strong interest in lifestyle medicine and utilizing lifestyle-based interventions as part of chronic disease management and preventive care
  • Be local to the area or have strong familiarity with the community
  • Bring experience from multiple healthcare settings
  • Be comfortable treating both adult and pediatric patients
  • Have experience managing pediatric medical concerns beyond routine wellness care
  • Embrace a lifestyle medicine approach, emphasizing nutrition, exercise, behavior modification, and preventive strategies alongside traditional treatment plans
  • Enjoy building long-term patient relationships and promoting overall wellness
