Job Summary
Advanced Practice Provider/Nurse Practitioner to provide longitudinal primary and preventive care for older adults in community-based settings across Boston, MA. APPs manage a dedicated patient panel as their longitudinal primary care provider, operate with high clinical autonomy, collaborate with an interdisciplinary care team and Medical Director, and deliver care through telehealth and in-person visits. Responsibilities include preventive and chronic disease management, care coordination, accurate EHR documentation, and supporting patient-centered care in aging communities.
Required Qualifications
- Active, unrestricted Nurse Practitioner or Physician Assistant license
- Board Certification: NP (AANPCB or ANCC — family, adult, geriatric, or adult-gerontology) or PA (NCCPA)
- Active DEA license and controlled substance registration where applicable
- 3+ years of clinical experience as an APP
- Basic Life Support (BLS) certification
- Valid driver’s license and reliable transportation (car stipend provided)
- Experience with EHR documentation (athenahealth experience preferred)
