Job Summary
Advanced Practice Provider/Nurse Practitioner to support older adults across Nashville, TN in a fully community-based model with no traditional clinic home base. Manage a dedicated patient panel as longitudinal primary care provider, collaborating with interdisciplinary team and Medical Director. Responsibilities include providing longitudinal primary and preventive care for older adults, growing the patient panel within assigned communities, delivering patient-centered care in community settings, supporting chronic disease management and care coordination, utilizing telehealth and in-person delivery, and maintaining accurate EHR documentation. Strong emphasis on autonomy, population health, value-based care, and commitment to equitable, patient-centered care. Requirements include NP/PA license, board certification, DEA license where applicable, 3+ years APP experience, BLS, driver’s license with car stipend, and Athenahealth/EHR experience; telehealth experience and geriatrics/family/internal medicine background preferred.
Required Qualifications
- Active, unrestricted Nurse Practitioner or Physician Associate license
- Board Certification: NP (AANPCB or ANCC — family, adult, geriatric, or adult-gerontology) or PA (NCCPA)
- Active DEA license and controlled substance registration where applicable
- 3+ years of clinical experience as an APP
- Basic Life Support (BLS) certification
- Valid driver’s license and reliable transportation
- Experience with EHR documentation (athenahealth experience preferred)
- Commitment to equitable, patient-centered care
