Job Summary
Advanced Practice Provider/Nurse Practitioner to deliver longitudinal primary and preventive care for older adults in community-based settings across Chattanooga, TN. APPs manage a dedicated patient panel with high clinical autonomy, collaborating with an interdisciplinary team and Medical Director. The role combines in-person community visits with telehealth delivery, focusing on aging/geriatric populations, care coordination, chronic disease management, and equitable, patient-centered care. Requires active NP/PA licensure, board certification, DEA license, BLS, and a valid driver’s license. Expect EHR documentation (athenahealth preferred) and a supportive, mission-driven environment with potential car stipend and benefit-rich package.
Required Qualifications
- Active, unrestricted Nurse Practitioner or Physician Associate license
- Board Certification: NP (AANPCB or ANCC — family, adult, geriatric, or adult-gerontology) or PA (NCCPA)
- Active DEA license and controlled substance registration where applicable
- 3+ years of clinical experience as an APP
- Basic Life Support (BLS) certification
- Valid driver’s license and reliable transportation (car stipend provided)
- Experience with EHR documentation (athenahealth experience preferred)
