Job Summary
Deliver full-spectrum primary care to patients of all ages, managing chronic conditions, preventative care, and acute health concerns. Work closely with physicians and clinical staff to ensure coordinated, high-quality care while maintaining accurate and timely documentation in the electronic medical record. Provide patient-centered care and health education within a collaborative, community-focused setting. This full-time day shift role is based at the Prime Healthcare Illinois Medical Group Family Practice clinic in Evanston, Illinois.
Required Qualifications
- Licensed NP or PA in the state of Illinois
- Board certification in Family Medicine (NP) or appropriate PA certification
- Strong interpersonal and clinical skills
- Commitment to patient-centered care and health education
