Job Summary
Provide in-home, on-site, and virtual primary care services including preventive care, chronic disease management, and acute care in collaboration with physicians. Develop individualized care plans and communicate effectively with patients, families, and the care team to ensure seamless coordination. Maintain accurate and timely medical documentation within the EMR and participate in team meetings, case reviews, and quality improvement initiatives. Assist with care coordination by facilitating appointments and transferring referral information. Requires an active Washington NP or PA license with at least five years of primary care experience, an unrestricted driver's license, and strong clinical skills in preventive, chronic, and acute care. Full-time compensation ranges from $110,000 to $150,000 with malpractice coverage and flexible scheduling.
Required Qualifications
- Active Nurse Practitioner (NP) or Physician Assistant (PA) license in the state of Washington
- Primary care experience
- Multiple state licensed
- Minimum of 5 years of experience in primary care
- Unrestricted driver's license
- Strong clinical skills in preventive care, chronic disease management, and acute care
- Exceptional communication and interpersonal skills
- Ability to work effectively in a collaborative, team-oriented environment
- Passionate about patient care and making healthcare more accessible
- Internet access
- A computer or laptop (Mac or Windows)
Desired Qualifications
