Advanced Practice Provider (NP or PA)

Conduct thorough patient assessments and develop individualized care plans in a post-acute care setting. Requires an active New York license as a Nurse Practitioner or Physician Assistant with previous experience in skilled nursing, long-term care, and rehab.
