Job Summary
Advanced Practice Provider (Nurse Practitioner/Physician Assistant) to deliver in-home and virtual, evidence-based care for patients with CKD/ESRD, conducting comprehensive assessments, counseling families, documenting medical histories and plans, coordinating care with primary and specialty providers, and ensuring timely follow-up. Role emphasizes autonomous practice in-state with remote in-home responsibilities, hybrid scheduling, and participation in an integrated care team. Requires active NP/PA license, board certification, DEA, and 2+ years of patient care; bilingual English/Spanish highly preferred. relocation assistance offered; compensation aligns with MGMA guidelines; includes comprehensive benefits and 401(k).
Required Qualifications
- Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license
- Board certified for appropriate licensure (NP: ANCC/AANP; PA: NCCPA)
- Current and unrestricted DEA certificate
- 2 or more years of direct patient care required
- Managed Care/IPA/Health Plan experience
- Experience conducting annual wellness visits or similar comprehensive visits virtually or in the home
