Advanced Practice Provider

Conduct comprehensive patient evaluations, including history-taking, physical examinations, and diagnostic data analysis to diagnose and treat acute and chronic conditions. Prescribe medications, manage care plans, and coordinate referrals with specialty providers while educating patients on self-management. Maintain accurate EMR documentation, adhere to Local MD charting standards, and ensure compliance with HIPAA and regulatory requirements. Provide medical direction and training to clinical team members, mentor staff, and collaborate on quality improvement initiatives. Build positive rapport with patients and staff, travel within a 25-mile radius, and maintain required licensure and certifications.
