Job Summary
Advanced Practice Provider role delivering direct ambulatory primary care to patients at Mazzoni Health Center. Responsibilities include taking comprehensive medical histories, diagnosing and treating acute/chronic conditions, preventive care, office-based procedures, patient education, coordinating care with team members, and participating in after-hours on-call rotations. Requires supervisory/collaborative physician agreements, ongoing documentation in EHR within 72 hours, and compliance with quality measures. Must maintain licensure (RN and CRNP or PA), a DEA license, and applicable background clearances. Employment emphasizes LGBTQ+ health, gender-affirming care, and lifelong learning with CME commitments; strong communication and collaboration across multidisciplinary staff are essential.
Required Qualifications
- Graduate of an accredited nurse practitioner or physician assistant graduate program
- Active licensure as registered nurse and certified registered nurse practitioner OR active licensure as a certified physician assistant in Pennsylvania
- Active DEA license
- Active supervisor/collaborative physician agreements in Pennsylvania PALS as required by licensure
- Primary care experience
- Experience in gender-affirming care, sexual health care, HIV care preferred
- Technological literacy with EHR and basic computer skills
- Commitment to LGBTQ+ community and inclusive environment
- Excellent oral and written communication skills
- Ability to work with patients, staff, and others in a respectful and professional environment
- Background checks: Criminal, Child Abuse, FBI in PA and residence state
- Vaccinations and physical requirements compliant
