Job Summary
Accountable for the provision of primary care medical services within a Patient-Centered Health Home context, including direct patient examinations, documentation of visits, and management of chronic and acute illnesses. The role requires effective communication, adherence to clinical protocols, and the ability to educate patients. The Advanced Practice Provider must meet budgeted visits, clinical care measures, and engage in performance improvement efforts. Required qualifications include a California nursing or physician assistant license and a master's degree or equivalent training.
Required Qualifications
- Licensed in California as a Nurse Practitioner
- Licensed in California as a Physician Assistant
- Extensive formal training as a Nurse Practitioner or Physician Assistant
- Significant experience in primary care medical services
Desired Qualifications
- Ability to create complex documents in Microsoft Word
- Advanced Microsoft Excel functions experience
- Development of sophisticated presentations in Microsoft PowerPoint
