Job Summary
Assess, diagnose, and manage treatment plans for patients with Alzheimer's disease and other memory disorders, including ordering and interpreting diagnostic studies and prescribing pharmacologic interventions. Provide health promotion counseling and education to patients and families while collaborating with a multidisciplinary team to develop individualized care plans. Practice with high autonomy in an independent outpatient clinic setting, maintaining accurate electronic medical record documentation and compliant coding. Participate in quality, safety, and research initiatives to advance dementia care standards. Monday-Friday, 40 hours/week.
Required Qualifications
- Active RN, and APRN/APRN-FPA/APNP or other APRN license(s) in state(s) of practice
- Active national board certification in area of clinical practice and populations served
- Active DEA registration prior to hire
- Active Illinois Controlled Substance License prior to hire (if Illinois practice)
- Active BLS and/or ACLS, PALS, NRP as required by clinical practice prior to or within 6 months of hire
- Master's Degree in Nursing
- Must be able to stand, walk, bend, stoop, and twist continuously throughout the workday
- Must have functional speech, vision, touch, and hearing
- Must be able to lift up to 50 lbs from floor to waist
- Must be able to lift up to 20 lbs over the head
- Must be able to carry up to 40 lbs a reasonable distance
- Must be able to operate all equipment necessary to perform the job
