Job Summary
Deliver patient-centered primary care services across the lifespan, including assessments, diagnoses, treatment planning, and medication management for underserved populations. Conduct initial health assessments, well-child visits, immunizations, and annual wellness visits while promoting disease prevention through patient education and screenings. Collaborate with an integrative health care team by participating in regular meetings, quality improvement initiatives, and case conferences. Document all patient interactions accurately in the Electronic Health Records system and ensure timely follow-up on labs, imaging, and specialist referrals. Work under limited supervision with a Monday through Friday schedule in a rural community clinic setting.
Required Qualifications
- Master's degree in Nursing or in Physician Assistant Studies from an accredited program
- Current certification through a nationally accredited Physician Assistant or Nurse Practitioner program
- Active and unrestricted NP / PA California license
- Current BLS / ACLS certification
- Current drivers' license
- DEA License
- Strong clinical skills (ability to diagnose, treat, and manage patient care)
- Intermediate language skills (ability to read, write and interpret routine documents)
- Basic math skills (ability to perform basic math required for patient care / medication management)
- Clinical judgement / reasoning (ability to perform high level reasoning in different clinical situations)
- Good working knowledge of standard medical equipment
- Excellent interpersonal and communicate skills to work with diverse populations
- Proficiency in EHR systems
- Basic working knowledge of Windows operating systems (Word, Excel, Outlook and internet browsers)
- Ability to stand, walk, and sit for extended periods of time
- Requires manual dexterity and visual acuity to perform medical examinations and procedures
Desired Qualifications
- Prefer at least one year experience in a clinical setting
- Experience in rural community clinic preferred
- Strong leadership skills beneficial
