Job Summary
Advanced Practice Provider role in a hybrid inpatient/outpatient advanced heart failure team at Rhode Island Hospital. Performs independent outpatient visits and manages a panel of complex heart failure patients (including LVAD and transplant patients) with shared visits with heart failure cardiologists. Conducts inpatient heart failure consults, interprets echocardiograms, catheterizations, MRIs, and stress tests; apply ACC/AHA heart failure guidelines; provides patient and family education; participates in weekly heart failure team meetings; collaborates with the heart failure team and hospital care team; maintains on-call pager for heart failure consults; requires leadership in care planning and coordination within a multidisciplinary cardiovascular team.
Required Qualifications
- Graduated from an accredited physician assistant program with a master’s degree and board certification from the NCCPA OR graduated from an accredited nurse practitioner master’s degree program with board certification from ANCC or AANP
- Current Rhode Island state licensure to practice as an APP or physician assistant
- Current prescriptive privileges and DEA licenses
- Current BLS and ACLS certifications
