Job Summary
Conduct acute care consultations for hospitalized patients and evaluate them to determine the most appropriate level of post-acute care, such as inpatient rehabilitation, skilled nursing facilities, or home therapy services. Assist the acute care rehabilitation specialist with inpatient consultations and collaborate with multidisciplinary teams to coordinate patient transitions. Perform comprehensive patient assessments, functional evaluations, and discharge planning while coordinating care with physicians, therapists, and case managers. Work primarily at Mary Free Bed Hospital with weekend rotations of approximately 7-8 times per year.
Required Qualifications
- Certified and licensed as a Nurse Practitioner (NP) or Physician Assistant (PA) by the State of Michigan
- Master's Degree in Nursing or Physician Assistant
- 3-5 years of experience with neurologically impaired patients
- Current Basic Life Support (BLS) certification
- Work primarily at Mary Free Bed Hospital while providing consultation services for patients admitted to Corewell Health's acute care hospital in Grand Rapids, Michigan
- Schedule Full-time Monday through Friday
- Weekend rotation of approximately 7-8 weekends per year
