Job Summary
Conduct in-home longitudinal primary care for adult and geriatric populations, performing patient histories, physical examinations, and differential diagnoses. Order, perform, supervise, and interpret diagnostic and laboratory tests while prescribing pharmacologic agents and non-pharmacologic therapies. Provide therapeutic measures, education, and counseling to individuals, families, and groups, emphasizing health promotion and disease prevention. Collaborate with supervising physicians for uncommon or unstable conditions, ensuring proper documentation of all prescribed medications and refills. Maintain current clinical skills through required continuing education and adhere to state practice acts and ethical standards. Work per diem as needed with a minimum of three years of Advanced Practice Provider experience.
Required Qualifications
- Advanced Practice Nurse or Physician Assistant
- Minimum of 3yrs, working with the adult and geriatric population as an Advanced Practice Provider (Nurse Practitioner or Physician Assistant)
- Current knowledge and understanding of Primary Care
- Current knowledge and understanding of Chronic Care Management
- Current knowledge and understanding of Best Practices
- Home based Primary Care experience preferred
- Master's Degree - Nurse Practitioner or Physician Assistant
- American Heart Association BLS certification
- NJ-APN & NJ-RN or NJ PA License
- Valid State Driver's License with a clean DMV record
- Copy of recent Vehicle Insurance
- Per Diem - as needed
