Job Summary

Provide advanced nursing clinical care for patients in accordance with State and Federal rules, managing inter-professional teams and leading rounds to achieve quality outcomes. Conduct comprehensive assessments, including post-discharge visits and palliative care onboarding, while establishing treatment plans and determining when further evaluation is warranted. Collaborate with patients, families, and physicians to deliver intensive care management across client settings, ensuring compliance with Patient-Centered Medical Home standards and HEDIS measurements. Participate in 24/7 on-call coverage and perform procedures per collaborative practice agreements. For Psychiatric Nurse Practitioners, conduct psychosocial assessments, provide psychoeducation and psychopharmacology, and deliver brief psychotherapy. Maintain professional certification, licensure, and documentation standards while supporting staff through peer reviews and precepting students.

Required Qualifications

  • License and current registration to practice as a Registered Professional Nurse in New York State
  • Certificate (license) and current registration to practice as a Nurse Practitioner in the State of New York, with a specialty in adult health, family health or gerontology
  • Valid driver's license
  • Maintains credentialed status with VNS Health Medical Care at Home and associated managed care plans
  • Maintains NPI, Medicaid and Medicare provider numbers
  • Must be certified by ANCC or another accrediting Nurse Practitioner body
  • Master's Degree of Science in Nursing, or other graduate degree from a nurse practitioner educational program registered by the New York State Education Department as qualifying for NP certification (licensure)
  • Current ANCC or AANP certification as an adult, family or geriatric nurse practitioner
  • Minimum of two years of experience as a nurse practitioner utilizing full scope of practice
  • Clinical home care experience or two years managerial experience
  • Demonstrated knowledge of Hedis and QARR quality measures, ICD-10 and CPT coding for reimbursement of services
  • Bilingual skills
  • Minimum of two years of experience as a psychiatric nurse practitioner utilizing full scope of practice

Desired Qualifications

  • Maintains a collaborative practice agreement with a physician in compliance with New York State regulations
  • PhD in psychiatric-mental health Nurse Practitioner
  • Minimum of two years of experience as a nurse practitioner utilizing full scope of practice preferred
  • Clinical home care experience or two years managerial experience preferred
  • Maintains NPI, Medicaid and Medicare provider numbers preferred
