Job Summary
Advanced Practice Clinician (Nurse Practitioner) provides full-scope advanced nursing care and clinical leadership for targeted patient populations, evaluating responses to therapy, revising care plans, and developing protocols in collaboration with physicians and the care team. Responsibilities include comprehensive physical examinations, history-taking, preventive and therapeutic interventions, care management across settings, discharge planning, and patient education in accordance with state and federal nurse practitioner scope of practice and national standards. The role requires bilingual proficiency in Mandarin/Cantonese, NY NP licensure, ANCC/AANP NP certification, and the ability to work within a patient-centered medical home framework while supporting quality measures such as HEDIS and QARR.
Required Qualifications
- License and current registration to practice as a Registered Professional Nurse in New York State
- License and current registration to practice as a Nurse Practitioner in the State of New York with specialty in adult health, family health or gerontology
- ANCC or equivalent certification as nurse practitioner (adult, family or geriatric)
- Current ANCC/AANP certification for NP
- PhD in psychiatric-mental health NP preferred for PMHNP track
- Current Master's Degree in Nursing from NYSED-qualified NP program
- Minimum two years of NP full-scope practice experience preferred
- Bilingual skills in Mandarin/Cantonese as operationally required
- Valid driver's license may be required
- Credentialed status with VNS Health Medical Care at Home and related plans preferred
- Maintains NPI, Medicaid and Medicare provider numbers preferred
- Collaborative practice agreement with physician preferred
- Experience with HEDIS and QARR quality measures, ICD-10/CPT coding preferred
- Active state NP license required
