Job Summary
Advanced Medical Assistant supporting the Maryland Primary Care Program and Value Based Contracts. Coordinates patient care, conducts patient outreach and education, conducts care coordination across the care team, and maintains knowledge of care coordination/management, CRISP/Value Based Portals, EPIC Registries, and patient self-management tools. Performs administrative and clinical tasks to ensure patients receive appropriate care, manages care gaps and utilization reports, schedules follow-ups, assists with SMART goal setting and self-management coaching, and collaborates with providers and care teams to improve outcomes and experience.
Required Qualifications
- High School Diploma
- Completion of accredited medical assistant training program
- CMA, RMA or AAMA (through an approved/accredited organization)
- CPR Certification
